Time Committment

  • You will complete two needs assessment questionnaires, taking between 20 and 30 minutes each. You will also be required to review the Amplify Care contract for data extraction, schedule the extraction, and provide access, which we estimate will take one hour.

    Next, reviewing your personalized report with insights is expected to take between one and two hours. It is recommended to establish a quality improvement (QI) plan for as many patients as practical (no minimum required), and this reflection exercise will take approximately one hour. If the QI plan requires a patient visit, this will be conducted as part of your routine clinical practice.

    Finally, you will be invited to participate in a 60-minute virtual "share the learning" session.

    Total estimated time commitment: Less than 6 hours

    In 2026, a second data pull is planned to validate previous quality improvement results, ensuring that interventions have led to measurable and sustained improvements in processes and patient care.

  • QUEST is designed to be minimally disruptive, requiring only limited touchpoints and efficient use of participants' time. The data extraction process is managed externally and performed after hours to ensure there are no disruptions.

  • The QUEST program will unfold across several key phases over the next six months:

    1. Recruitment & Onboarding (Begins August)
    Participant recruitment and onboarding begin in August. This step ensures all participants are informed and ready to engage in the quality improvement process.

    2. Data Extraction & EMR Analysis (Ongoing)
    Once participants are onboarded, data extraction from electronic medical records (EMRs) and subsequent analysis begins. This process is managed by Amplify Care. For more information, read our Data Extraction page.

    3. Distribution of Individual Reports with Insights & Aggregate Data (Ongoing)
    Participants will receive personalized reports that include both individual insights and aggregate findings. These reports support reflection and preparation for implementation planning.

    4. Individual QI Implementation Plans (August to November)
    From August through November, participants will create and begin applying their own Quality Improvement (QI) implementation plans based on the insights provided in the reports.

    5. Virtual Group Sharing Session (Scheduled for December)
    In December, participants will join a virtual group session to share experiences, challenges, and progress with peers.

    A second data extraction will take place in early 2026 to compare participant outcomes before and after the QI plan implementation.

Shaping Future Learning

  • You will receive compensation in the amount of $375.00 for completing the pre- and post-program needs assessment questionnaires.  The needs assessments will identify both the perceived and unperceived educational needs, ensuring that learning initiatives created following QUEST address real gaps in knowledge, skills, or performance.

Data

  • QUEST will assess the management of patients living with type 2 diabetes (T2D), particularly those with cardiovascular or renal comorbidities. It will identify both successes and challenges in care, focusing on variables aligned with the Diabetes Canada Clinical Practice Guidelines, including recent updates to the Pharmacotherapy and Chronic Kidney Disease (CKD) chapters.

    Data analyzed will include:

    • Total patients in practice

    • % of patients with T2D 

    • % of patients with T2D+eCVD 

    • % of patients with T2D+eCVD without SGLT2 or GLP1 

    • % of patients withT2D+CKD 

    • % of patients with T2D+CKD without SGLT2 or GLP1 

    • % of T2D patients without eGFR and uACR in the past year 

  • Amplify Care will manage the entire data extraction process, requiring minimal time from participants. The process is designed to be non-disruptive and can be performed after hours to avoid interfering with operations. Clearly defined search criteria will be used to generate detailed, personalized analyses for each participant.

  • No, the process is designed to ensure no disruption to participants or their electronic medical record (EMR) systems.

  • Data will be encrypted both in transit and at rest, stored exclusively on servers located within Canada, and destroyed within 30 days of program completion. Patient IDs are used to ensure anonymity and are not directly linked to patients outside the source system.

Legal

  • You will receive a contract for review and signature that clearly defines the search criteria, ensuring the search remains strictly within these agreed parameters. The contract will also address confidentiality and data protection.

QI Initiative Details

  • Participants will have up to 4 months to complete their QI project. While there is no fixed number of patients required, we encourage you to include as many patients as is practical and meaningful within your clinical context. The goal is to generate insights that reflect real-world impact.

  • You’ll receive a Quality Improvement (QI) Tracker—a practical tool with dedicated sections for setting objectives, tracking interventions, and capturing lessons learned. This template supports ongoing management and serves as a valuable reference for future quality improvement initiatives.

Additional Information

  • You will have access to a dedicated QUEST team member throughout the initiative.

  • The virtual session aims to provide a safe, confidential environment for participants to share practice reflections, discuss challenges, and celebrate successes. All reporting and presentations are conducted anonymously to promote open discussion and collaborative learning that enhances care quality across practices.

  • If you are unable to attend the 60-minute virtual learning session, a link to the recorded session will be provided so you can still benefit from the discussions and insights shared by other participants.

  • Up to 50 nurse practitioners from across Ontario will participate.
